Clinical assessment and rehabilitation
For case managers, healthcare teams and other authorised referrers seeking practical support for an adult’s everyday function. Referral questions may concern cognition, fatigue, routines, equipment, environmental barriers, participation or vocational function.
The assessment and rehabilitation plan is agreed around the person’s goals. Reporting, review points and communication with the referring team are defined before work begins.
Independent expert witness instructions
For solicitors and other authorised commissioners seeking independent opinion. Connor considers the functional impact of injury or disability and relevant rehabilitation and support needs, within his expertise.
A clinical referral is not automatically an expert instruction. Conflicts, questions, jurisdiction, timetable and terms are checked before acceptance.
Making an initial enquiry
Please provide your name and organisation, the service requested, broad referral questions, assessment region and important dates. Do not paste medical records or detailed client information into the website form. Further information and conflict identifiers will be requested through an appropriate route.
Once suitability is established, we confirm scope, availability, a private quotation, terms and secure records transfer.
Communication and reporting
We aim to acknowledge and review your enquiry by the end of the next business day, Monday to Friday excluding bank holidays.
Reports are normally provided within 10–14 working days from the initial assessment. Any different deadline or change to the agreed timetable will be discussed and confirmed in writing.
Business and free-call availability is 08:00–20:00, Monday to Sunday, by appointment and subject to availability. The enquiry response target is measured in business days.
Self-funded rehabilitation blocks and supervision packages are paid in advance; other invoices are normally payable within 30 days unless otherwise agreed in writing. ProActive Therapy is not VAT registered.
Professional referral FAQs
Can employers, insurers and public bodies make enquiries?
Yes. Enquiries are welcomed from authorised commissioners as well as case managers, solicitors and healthcare professionals. Scope, funding and the appropriate professional role are agreed before work begins.
Can a report be prepared for a particular organisation or purpose?
Yes, where the purpose, intended audience, questions and deliverables are agreed in advance. A treating or rehabilitation report is different from independent expert evidence.
Does submitting an enquiry reserve an assessment or report date?
No. Dates and deadlines are reserved only after written acceptance, following the relevant suitability, conflict, capacity and terms checks.
What funding information is needed before work starts?
The payer, any third-party funding and any purchase-order requirement must be confirmed. A written quotation and applicable terms are agreed before substantive work begins.
