Expert-witness and professional enquiries
Use the professional form for an initial suitability and conflict-check enquiry. Provide only the minimum information requested. Do not include medical records, reports, detailed health information, bank details or other sensitive documents.
After the initial check, ProActive Therapy will confirm whether the proposed work can be accepted, what further information is needed and how records should be transferred securely. An enquiry is not an accepted instruction, and no assessment, deadline or court date is reserved until written acceptance.
Response target: ProActive Therapy aims to acknowledge and review the initial enquiry by the end of the next business day after receipt, Monday to Friday excluding bank holidays. Scope, availability, fees and any timetable are confirmed in writing before work is accepted.
Self and family occupational therapy enquiries
Use the self and family form for a general discussion about assessment or rehabilitation. Share only a brief description of the support requested. Detailed health records should not be uploaded through the public website.
Response target: ProActive Therapy aims to acknowledge and review the initial enquiry by the end of the next business day after receipt, Monday to Friday excluding bank holidays. Suitability, availability, fees and the earliest appointment window are confirmed before a clinical appointment is booked.
